As for technique? Night photomanipulations are always hardest for me personally. You really have to work double duty on light and shadows and if you don't.. the scene ends up looking drab, unbelievable, and flat. I think more work with light and shadows could have been done on the railing and arches where moon shines through. The same goes for the bride. Since she's turned away from the camera, it's assumed that the moonlight would catch across the shoulder and part of the inner-arm. The absence of the lighting and extra shadowing really pulls away from the idea that the scene is taking place at night.
The blood is a little faded and flat on her dress. It looks like just a light red brush was used. I like to use the burn and dodge tools to add a little dimension to liquids like blood. Lastly, the cake's positioning bothers me. It looks like it's hovering above her dress and if you repositioned it, it would look infinitely better. I'd suggest putting it on a small table or maybe remove the drop shadow, paint a little bit of her dress over the base of the cake (so it looks like it's actually sitting in the cloth).
Despite these imperfections, I think this really came together well. You put an interesting story within the piece and in the age of photomanip themes like "Lonely girl standing in a field", it really is refreshing to see a lot of thought put into detail.
